To place an order please fill out the form below and submit payment. Each order cost $40. With each order you will receive one 2"x2" QR code that directly links you to a webpage created for your loved one. There are options to purchase more QR codes with your webpage located within the form below.

The form below ask for information in regards to your loved one along with a spot for 10 photos and 1 video for upload. Have your photos and video readily available at the time of filling out the form. Once the form is submitted there is no opportunity to add, edit, or delete.

Due to the fact that each QR code and webpage are customized, changes or edits once submission has occurred is a request that cannot be fulfilled. Along with the inability for changes to occur, all sales are final and are unable to be refunded. Please ensure orders are filled out to the best of one's ability and do not hesitate to email hereaftermemories@gmail.com for further information before final submission.
